Free forms available to meet OCR requirements


SIOUX CITY, IOWA (June 15, 2017)—Northwest AEA and the Iowa Department of Education is pleased to partner with TransACT Communications ( to provide a collection of district and school forms, notices and letters.  These forms, notices and letters are free for district and school staff to use to help meet Office for Civil Rights (OCR) requirement to communicate with parents and families. This is open to ALL staff!
This easy-to-use online service includes:
GenEd Parent Notifications: Contains more than 100 documents, forms, and letters for reaching out to the parent community. Available in up to 21 languages.

  •   Health and Medical – Head Lice, Head Injury, Immunizations
  •   Free and Reduced Meals – Letter to Households, Application
  •   School Administration – Back to School KitsBullying formsParent Teacher Conference Letter, Behavior Referral, Bus/Field Trip, etc.
    1.   Campus Signs – Bully Free Zone, Welcome to Our School, Restrooms, Office, etc.
  •   Special Services –Title III/ELL forms, Section 504 Parent-Student Rights, etc.

ESEA/NCLB Parent Notifications: Contains more than 70 notices supporting title program mandates under the NCLB Act (Title I, III, Highly Qualified Staff, Homeless).  Available in English
